Health Professionals Undertake Care / Support work with the sensitivity required to provide services in a way which will preserve the dignity, privacy, choice, independent, fulfilment and rights of service Users and their carer / family members.
Main Responsibilities:
1. To undertake personal care or support task as outlined in the Service Users Care/Support Plan
2. To maintain good communication and develop effective working relationships with Service Users.
3. Liaise with nursing homes staff and/ professionals as necessary or as directed
4. To adhere to all of the client policies and procedures
5. To alert Field Care Supervisors/Managers of any changes in the Services Users condition or circumstances
6. To develop good communication links with any Carer or family members in the Service Users, whilst maintaining professional boundaries.
